The Relational Practices Ltd. includes psychologists, clinical social workers, couple and family therapists, and support staff. We provide psychological care and treatment through our practice. Sometimes, we work with outside consultants or agencies who may need limited access to your health information to help with your care.

What is Personal Health Information?

Personal health information includes details that can identify you and your health care, such as:

  • Contact details (name, address, phone number, email)

  • Personal characteristics (age, gender, ethnicity)

  • Health information (issues you're facing, medical history, treatments)

Collection and Use of Personal Health Information:

We collect your personal health information to:

  • Understand your health needs

  • Recommend suitable treatments

  • Provide therapy and related services

If third-party payers (like insurers) are involved, they might ask for specific information, such as:

  • Session dates, duration, and provider details

  • Information on health conditions, treatment needs, and progress for funding decisions

Information Sections:

Email Communication: We may use email for appointment reminders and invoices. Email is not fully secure, so it has risks like being intercepted or misaddressed. We cannot guarantee complete security but use reasonable measures to protect your information.

Tele-Services: We offer virtual (video) and in-home sessions. Virtual sessions are done through a secure platform. Your clinician will discuss the best method for you, and you can change it anytime. Virtual sessions also have privacy risks, and confidentiality rules still apply.

Disclosure of Personal Health Information:

We keep your information confidential unless:

  • There is a risk of harm to yourself or others

  • There is suspected abuse or neglect

  • A court orders us to provide information

      

Access to Your Personal Health Information & Requests for Correction:

You can request to see or correct your health information. If we cannot provide access, we will explain why in writing.

Storage and Destruction of Personal Health Information:

We keep your information secure for six years after your last contact. After this period, your information is securely destroyed.
